It’s very strange to just buy a pair of jeans that fit. When I would do late-night, my biggest concern wasn’t my set – it was, did I look fat?

These persistent insecurities trace back to an experience during the production of the 1992 film A League Of Their Own. Director Penny Marshall reportedly requested that O’Donnell lose twenty pounds for her role, a comment that resonated with her for decades.

When I look back, I go, ‘God, I wish I could have enjoyed the size I was’. I’m now at that same weight. But I feel much healthier, more powerful in my own physicality.

The Facelift Decision

Beyond her medication journey, O’Donnell underwent a deep plane lower facelift, a procedure she first mentioned in a Substack post during May. In that earlier disclosure, she discussed feelings of guilt and shame surrounding the cosmetic enhancement.

I lost so much weight that I had a lot of excess skin (around my chin), and it made me look sad. I didn’t want my face to be frozen. I had to wrestle with myself because as a feminist I always felt this is the male gaze and not anything I believe in.

Her youngest son Clay played a pivotal role in her decision-making process, reminding her that younger women look up to her as a role model. After six months of deliberation, she concluded that true feminist empowerment means having autonomy over one’s own body choices.

I changed my mind for six months and then decided the real power of feminism (is) to decide what we want to do with our bodies and when.

She kept the procedure private until people began commenting on her improved appearance. The investment totaled ninety thousand dollars, a sum she considered substantial but worthwhile, noting she would never allocate that amount toward purchasing a vehicle.
